Home risk assessment, nursing care, implementation/ documentation of plan of care(POC) to ensure quality and continuity of care, collaborates with interdisciplinary team, physicians, patient and patient's family to plan care and evaluate outcome initiates appropriate preventive and rehabilitative nursing procedures, prepare clinical progress notes, and coordinates services, conduct health awareness programs, performed supervisory visit for LVN team members to provide Quality Assurance in regards to patient plan of care,and comp regulation compliance, provides education to non compliant and knowledge deficient regarding disease process, medication management, diet and exercise regimes, ensuring integration of safety and quality standards into practice and delivery of team care.
